السلام عليكم ورحمة الله وبركاته
اليوم نشرح لأعضاء المنتدى كيف نقيس ضغط الدم .
اي واحد يريد يتعلم انا جاهز يفرغ نفسة ربع ساعة فقط .
اول شي لا اريد الدخول في الاسباب والمسببات .
الأعراض . في ناس يعتقدون النزيف من الانف والغثيان هم من الأعراض ؟؟ وانا اقول هذا في حالة يكون فيها الانسان وصل الى مرحلة الخطر الكبير .
عموماً اذا تبون الشرح يطول وانا اريد استسهل الامر عليكم . ولا ادخل في مصطلحات طبية معقدة .
ضغط الدم..

هو عبارة عن قوة اندفاع الدم المسلطة على الشرايين الناتجة من انقباض القلب
نستطيع ان نحس بذلك عند قياس النبض ... تعرفون تقيسون النبض مو ؟

قياس ضغط الدم الطبيعي ((120_80))..
واذا كان ضغط الدم في حاله سكون الجسم ((140_90))..او اكثر يكون هذا الشخص مصاب . ولكن الأكثرية لا يعرفون ذالك لان الأعراض تكون بسيطة وهذا خطر كبير قد يؤدي الى السكتة الدماغية لا سامح الله.

في اسباب كثيرة
طبعا يختلف من شخص الى اخر معتمد على عدة عوامل العمر. الجنس. طبيعة ونوع العمل . الحالة الصحية . التدخين. تناول الكحول والمخدرات ... الجنس. الرجال يصابون اكثر من النساء بسبب القهر واللون كذالك وكما قلت قبل قليل لا اريد اطيل عليكم .


يوجد ضغطان للقلب الضغط الانقباضي (( وهو ما نسميه بالعامية العالي او المرتفع ))





والضغط الانبساطي للقلب (( وهو مانسميه الواطي ))





ومعدل الضغط الطبيعي يكون
( العالي ) 110 ملم زئبق على 70 ( الواطي ) وهذا مقبول




( العالي ) 120ملم زئبق على 80 ( الواطي ) وهذا جيد جدا وهو ضغط مثالي
( العالي ) 130 ملم زئبق على 90 ( الواطي ) وهذا على الحافه يحتاج مراقبة بين فترة واخرى






اعراض ضغط الدم..




الصداع ( عادة في مؤخرة الرأس وخاصة عند الاستيقاظ في الصباح ) ، و الدوار أو الدوخة ، ومع ذلك فالصداع غالبآ ما يكون خفيفآ وبالتالي يهمله المريض ويتطور

اعراض انخفاض ضغط الدم...




دوخة عند الوقوف وخمول والرغبة الى عدم عمل مجهود وتختلف الحالة حسب عمرالمريض او اذا كان لدية اعراض اخرى مصاحبة مثل امرض القلب فقر الدم نقص السكر فىالدم كذالك بعض الأدوية تسبب ذالك ...
وخاصتاً النساء عقاقير منع الحمل وغيرها







الأن نجي لطريقة الفحص ..






راح نشرح المبدا لقياس الضغط مهما كان نوع جهاز الضغط اذا زئبقي واذا ابو الساعة واذا الكتروني المبدا واحد






الادوات المطلوبة





جهاز لقياس الضغط مع سماعه طبية
سماعتي (( لا تخافون عني سماعتين ))ههههه







نقوم بلف الجلد المرفق مع جهاز الضغط على الذراع اليسرى او اليمنى ويفضل اليسرى فوق نقرة الذراع باربع اصابع





(( حدود الوضوء )) كما مبين بالصورة رقم واحد (1)














نضع السماعة في نقرة الذراع ( لان هذا المكان تكون الشراين قريبة ) وبعدها نقوم بنفخ الجلد المربوط بواسطة






الكفة ( بم زغير) المرفقة بجهاز الضغط بالضغط المتناوب عليها سوف نسلط بهذه العملية ضغط على الشرايين اكبر من ضغط القلب لذا سوف يتوقف سريان الدم ضمن الشرايين نسبيا نراقب موشر جهاز الضغط زئبقي او ابو الساعة






سوف نجده اكبر من 140 او 150 بحيث لانسمع أي صوت بعدها نقوم بتخفيف



كما بالصورة اسفل













ضغط الهواء المسلط على الذراع برخي الصامولة المرفقة بالكفة نسبيا وببطئ شديد سوف نلاحظ ان موشر جهاز الضغط يبدا بالنزول وهنا نراقب السمع وبشدة





سنلاحظ سماع ضربات تناغمية مع ضربات القلب ( بدا الدم بالسريان عبر الشرايين ) نلاحظ سماع الضربة الاولى عند أي رقم مثلا 120 هذا هو بداية الضغط الانقباضي ( العالي )













ونستمر بالعملية ونلاحظ سماع اخر ضربة عند أي رقم هذا هو الضغط الانبساطي ( الواطي ) ولنفرض 80 كما بالصورة اسفل













لذا نقول للشخص ان ضغطك 120 على 80 ونقارن هذه النسبة بالنسب الطبيعية سوف نجدها ضمن الطبيعي





نكرر العملية بعد فترة للتاكد .





ملاحظات مهمة ..؟؟؟





1- يجب ان يكون جهاز الضغط بمستوى الذراع لا اعلى ولا اسفل .اوكي



2- الضغط الجيد على السماعة وبالمكان المناسب ووضعها على الاذن بشكل جيد.



3- اعطاء فترة استراحة للمريض قبل قياس الضغط .



4- اعادة الفحص مرة ثانية وبعد فترة من الفحص الاول للتاكد .



5- يجب ان تكون اذني الشخص القائم بالفحص سليمتان .



اما بالنسبة للاجهزة الحديثة فهي تقوم بالعمل اتوماتيكيا وتظهر النتائج على شاشة صغيرة













ولكن اغلب الناس لاترتاح لهذه الاجهزة وغالبا ماتحتاج لشخص مختص لابما ترتاح عندما تشكي له او يطمئنها بالكلام ( تاثير نفسي ) كبير صدقوني اكثر المرضة يشفون من اضحك معاهم وأكلمهم بلطف .
