اضطراب الوسواس القهري Obsessive Compulsive Disorder

هو اضطراب عصبي حيوي هام significant neurobiological disorder, من الممكن أن يعرقل بشدة مهام أكاديمية, واجتماعية, ومهنية. والسمة الأساسية لهذا الاضطراب هي تكرار الأعراض والعلامات, والتي تؤثر على الحياة اليومية للشخص الذي يعانى من هذا الاضطراب, وقد كان يعتقد من قبل أنه نادرا ما يصيب الأطفال والمراهقين, ولكن الآن يعتقد العلماء أنه يصيب نحو 2% من الأطفال, وتشير التقارير إلى أن هذا الاضطراب بين المراهقين يمر دون تشخيص الكثير من الحالات المصابة به, وبالتالي لا تأخذ هذه الحالات حقها من العلاج الذي هو الآن مؤثر وفعال, سواء كان في صورة عقاقير, أو علاج إدراكي سلوكي.

و اضطراب الوسواس القهري العصابي Obsessive-compulsive disorder هو نوع من القلق العصابي النفسي, والذي يشعر فيه المريض بأن لديه أفكار, و مشاعر و أحاسيس, و تصرفات, تجعلهم يشعرون أنهم منقادين لعمل شيء, وبمعنى آخر يكون لدى الشخص هواجس أو ما يسمى بالاستحواذ obsessions تضطره لفعل أشياء أو ما يسمى بالإلزام compulsions.

أسباب اضطراب الوسواس القهري
هذا الاضطراب منتشر أكثر مما كنا نعتقد, ومعظم الذين يصابون به يكونون في عمر الثلاثين عام, وتوجد نظريات لتفسير أسبابه, ولكن لم يثبت أحد هذه النظريات, وقد أشارت بعض التقارير إلى دور العامل الوراثي, وربطت بين إصابات الرأس, أو العدوى, في محاولة لتفسير أسباب المرض, وقد بينت بعض الدراسات وجود عيوب بالمخ عند الأشخاص المصابين, كما أن بعض العلماء أشاروا إلى أن نقص مستوى مادة السريتونين serotonin قد تكون ضمن الأسباب ولكن هناك حاجة لمزيد من الدراسات.

وحوالي 20% من المصابين باضطراب الوسواس القهري, يكون لديهم تقلصات لا إرادية متكررة بالعضلات, وهذا يجعل العلماء يقترحون أن هناك علاقة بين هذا الاضطراب ومتلازمة توريت Tourette syndrome, ومع ذلك فإن الصلة بينهما ليست واضحة. وقد اكتشف العلماء أن الأشخاص المصابين بهذا الاضطراب ليس لديهم التثبيط الكافي لمناطق في المخ والتي هي مسئولة عن إيقاف السلوكيات المتكررة.

تكرار الأفكار الاستحواذية و التصرفات الملزمة ليست بسبب مرض عضوي أو تعاطي عقاقير.
تكرار الأفكار الاستحواذية و التصرفات الملزمة يسبب محنة وتداخل مع الحياة اليومية.
محاولات المريض لإيقاف أو تجاهل الأفكار أو التصرفات المتكررة تكون فاشلة.
النظافة الزائدة خوفا من العدوى.
تكرار غسيل الأيدي أو تكرار العودة للتأكد من غلق الموقد أو غلق الباب.
تكرار التصرفات والكلمات.

و يرى العلماء أن المرض يستهلك أكثر من ساعة يوميا من وقت المريض.

و مثال لاضطراب الوسواس القهري العصابي, تكرار غسيل الأيدي بسبب الانشغال بالخوف من الميكروبات, وفى العادة فإن الشخص يعترف أن سلوكه مفرط وغير معقول. و يفسر العلماء التصرفات المتكررة للمرضى بهذا الاضطراب على أنها محاولة لإيقاف القلق الذي تسببه الهواجس أو التخلص منه.

فحوص و اختبارات اضطراب الوسواس القهري
وصف تصرفات المريض يساعد في التشخيص, و الفحص الطبي يمكن من استبعاد الأمراض العضوية, و التقييم النفسي يستبعد وجود اضطرابات عقلية أخرى. هناك مقياس يسمى ييل براون يساعد في تشخيص اضطراب الوسواس القهري, ويمكن من متابعة تقدم العلاج.

في العادة لا يحدث تطور إلى مرض آخر.

عندما تؤثر الأعراض على الحياة اليومية و العمل و العلاقات.

علاج اضطراب الوسواس القهري
تستخدم الأدوية في علاج الوسواس القهري, و أول الأدوية المستخدمة هي نوع من الأدوية المضادة للاكتئاب, وهى مثبطات إعادة امتصاص السيروتينين الانتقائية (selective serotonin reuptake inhibitor (SSRI. وتشمل هذه المجموعة:


سيتالوبرام Citalopram
فلوكستين Fluoxetine
فلوفوكسامينFluvoxamine
باروكستينParoxetine
سرتلين Sertraline

وإذا لم تؤثر هذه المجموعة, فإن الطبيب يصف دواء يسمى كلوميبرامين clomipramine, وهو من الأدوية القديمة المضادة للاكتئاب, وهو أيضا أقدم الأدوية المستخدمة لعلاج اضطراب الوسواس القهري وهو ذو كفاءة في علاج هذه الحالات, ولكن له آثار جانبية أهمها صعوبة البدء في التبول, و هبوط ضغط الدم عند الوقوف من الوضع جالسا, و جفاف الفم, و النعاس. و أحيانا يستخدم مثبط إعادة امتصاص السريتونين الانتقائي مع الكلوميبرامين. وقد تستخدم الأدوية المضادة للقلق مع الأدوية السابق ذكرها.

و يستخدم العلاج النفسي Psychotherapy للتقليل من الضغط النفسي, و لتقليل القلق, و حل الصراعات الداخلية. و يستخدم العلاج السلوكي Behavioral therapies وذلك بتعريض المريض مرات متعددة للمواقف التي تثير قلقه, و تعليمه مقاومة الحافز المرضي الذي يضطره للتصرف المرضي, كما يشمل العلاج السلوكي إيقاف الأفكار الغير مرغوب فيها و تركيز الانتباه على تخفيف القلق.

ويعتبر اضطراب الوسواس القهري من الأمراض طويلة الأمد, و المزمنة والتي بها فترات كمون و فترات نشاط للأعراض, و الفترات التي ليس بها أعراض تكون غير معتادة, كما أن معظم الأشخاص يتحسنون بالعلاج.

من الممكن أن تساعد جراحة المخ و الأعصاب المرضى المتأثرين بالمرض بصورة شديدة, وذلك بعد فشل وسائل العلاج الأخرى و مثال هؤلاء, الذين تكون حالاتهم مسببة لتدهور العلاقات الأسرية, أو الذين ينقطعون عن العمل, أو الوظيفة الاجتماعية, والذين يحدث لهم خراب للاقتصاد الشخصي بسبب هذا الاضطراب, والذين ليس لديهم القابلية لاستخدام النقل العام بسبب شدة الانشغال بالخوف من العدوى, والذين تزداد لديهم المشاكل النفسية.
